AbbVie Has Quietly Changed the Subject — From Immunology Combos to Myeloma

A third Skyrizi raise and a 5.2% dividend hike grabbed the headline; the keyword rotation and a courtroom win over Botox IRA pricing say the next leg of the story is hematology.
ABBV · Earnings Call · 2026-10-30

The beat that stopped being news

AbbVie's third quarter was, by the numbers, another metronome beat: revenue of $17.4 billion, adjusted EPS of $3.82, $0.07 above the guidance midpoint, and a full-year revenue outlook lifted to roughly $67.8 billion — $800 million of cumulative raises since January. The dividend went up 5.2% to $1.82 a quarter. “Total net revenues were $17.4 billion, beating our expectations by $200 million and reflecting robust sales growth of more than 10%.” — Robert Michael, Chairman and Chief Executive Officer · 2026-10-30

What's changed is not the beat — AbbVie has been beating for a while. What's changed is what the market is paying for. The stock is up 21.3% over the last 90 trading days, essentially one uninterrupted seventeen-week advance, and roughly 24% year-to-date. That has compressed the forward yield to around 2.5%, the lowest since 2018. In a tape where Fed rate hike is the market's second-hottest global theme and long-bond yields are the reference anxiety, a 2.5% yielder does not rise 21% in a quarter on income appeal. AbbVie has stopped trading as a bond proxy and started trading as a growth asset.

The keyword ledger flipped sections

Look at where this company's own vocabulary has migrated. The top of the third-quarter keyword list is not immunology at all. It is R CHOP at number one, multiple myeloma at two, frontline DLBCL at four, with free survival and bispecific antibody close behind. A year ago this list was about HA fillers and dermal fillers; two quarters ago it was alpha-4-beta-7, the Apogee transaction and co-formulation work in IBD.

The content behind those labels is real, not narrative drift. Roopal Thakkar described the Phase III EPCORE DLBCL-2 result for Epkinly plus R-CHOP as “reducing the risk of disease progression or death by 51%” — Roopal Thakkar, Executive Vice President, Research and Development, Chief Scientific Officer · 2026-10-30 in a broad frontline population — the first bispecific combination to show a progression-free survival benefit in that setting — with submissions early next year. Etentamig, after a single step-up dose and monthly dosing thereafter, was filed in multiple myeloma with approval anticipated in 2027, and management is explicit that the convenience and low CRS rate are the differentiation versus CAR-T in community centres.

Meanwhile the old engine is not exactly sputtering. Subcutaneous induction in Crohn's is genuinely new this quarter — Subcu induction still ranks in the top twenty — and Jeff Stewart expects that after the roughly 60-day access cycle “we anticipate a pretty rapid share acceleration there” — Jeffrey Stewart, Executive Vice President, Chief Commercial Officer · 2026-10-30. Skyrizi guidance went to $21.8 billion, its third raise this year; Rinvoq to $10.3 billion. Rob Michael's framing is that the pair will add $6 billion combined this year despite competitive entry in both psoriasis and IBD. That is a striking claim for two products in year eight.

Third verse of the same song

There is a tension worth naming. Michael has now made the same argument three calls running. In February: “I see numerous sources of upside across the enterprise.” — Robert Michael, Chairman and Chief Executive Officer · 2026-02-04 In April: “we do see upside for the total company revenue in every year with that upside growing.” — Robert Michael, Chairman and Chief Executive Officer · 2026-04-29 And this week:

It's a multibillion-dollar opportunity that is significantly undercalled in many Street models. I would add Temab-A to that as well, similar a multibillion-dollar opportunity, significantly undercalled in many Street models.

Robert Michael, Chairman and Chief Executive Officer · 2026-10-30

Three consecutive quarters of "the Street is too low" is normally a tell — a management team negotiating with its own multiple. Two things make it different here. First, the company stopped publishing the granular long-term guidance that used to anchor consensus: the old targets, Michael said in April, served a specific purpose ahead of the Humira loss of exclusivity. Without them, the gap between management's internal view and sell-side models is structurally wider than it used to be. Second, and more importantly, the tape is now agreeing. The re-rating is the first external corroboration of a claim that had been purely rhetorical.

Worth flagging the odd man out in the keyword list: low rates ranks fifth this quarter and has nothing to do with interest rates — it refers to tolerability. Juvmo's edge in Parkinson's is low rates of dyskinesia, sedation and impulse-control disorder, the sleep attack problem that damaged the prior generation of dopamine agonists. Stewart says demand is running ahead of a Medicare window that only opens properly in 2028: “the high-level demand in the funnel has been exceeding our initial expectations.” — Jeffrey Stewart, Executive Vice President, Chief Commercial Officer · 2026-10-30 A $5 billion Parkinson's franchise is being priced before its coverage exists.

The two things that went quiet

Aesthetics is the tell. It was a top-three keyword theme through late 2025 — dermal filler, HA filler sentiment, filler market pricing. This quarter the segment did $1.2 billion, down 1.8% operationally, with Juvéderm off 11.8%, and no aesthetics term cracked the top thirty despite a full prepared-remarks passage. Guidance held flat at $5 billion. Boey's resubmission slips to year-end with a decision in mid-2027. A $5 billion franchise has become narrative furniture.

The other reversal is louder, and it is genuinely new. In February, Botox's selection for Medicare negotiation was a disappointment AbbVie had planned around: “we're obviously disappointed that it was selected given that it's a plasma-derived product and should have been excluded.” — Robert Michael, Chairman and Chief Executive Officer · 2026-02-04 This week Michael answered a question about IRA pricing with a court docket instead of a discount:

Botox has 100,00x more human serum albumin than toxin, which is why we believe Botox is not eligible for selection under the IRA and a Federal District Court just yesterday ruled in our favor.

Robert Michael, Chairman and Chief Executive Officer · 2026-10-30

Subject to appeal, but a dated, binary win that did not exist a quarter ago.

The denominator is doing the work

Here is where the fundamentals add discipline to the story. Trailing total revenue of $17.0 billion grew 10% year over year in the last filed quarter, with the operating margin at 37.9%, up 6.1 points. R&D ran at roughly 13.8% of revenue.

Yet the stock is not more expensive on earnings than it was a year ago. Price to operating income sits at 26.3x, down 12% year over year and 42% below its 2025 peak. That is the cleanest bull case available: AbbVie is being re-rated on the denominator, not the numerator.

The caveat sits in cash. Scott Reents noted “we generated approximately $14 billion of free cash flow in the first 9 months of the year” — Scott Reents, Chief Financial Officer · 2026-10-30, and the company reiterated a path to 2x net leverage in two to three years post-Apogee. But the last 10-Q showed a free cash flow margin of 17.4%, versus roughly 41% a year earlier, against $55.9 billion of net debt and interest coverage of 8.6x. If management wants to keep buying differentiated assets — the stated bolt-on strategy — cash conversion matters more than another guidance nickel.

So the story is this: AbbVie has spent three years proving Skyrizi and Rinvoq could replace Humira, and it has won that argument. The question the stock began paying for in July is what the next $5 billion looks like — and the answer is now coming from myeloma, frontline DLBCL, and a Parkinson's drug waiting on a 2028 coverage window. The keyword ledger flipped before the revenue line did. That is usually the sequence that precedes an estimate revision cycle.
