BD's New BD Delivers: Growth Platforms Accelerate, Headwinds Contained
The First Full Quarter of New BD
BD's fiscal third quarter marked the first full period as a focused medtech company, and the numbers landed ahead of plan. Revenue of $5.0 billion grew 4.4% on an FX-neutral basis, with adjusted operating margin at 24.9% and adjusted EPS of $3.23, up 4.9%. The acceleration is not accidental. Early results are tangible: roughly 100 to 150 basis points of APM's double-digit growth is attributed directly to those incremental investments. Contained Headwinds, A Raised Guide
The quarter's growth was checked by three known drags—Alaris, vaccines in China, and broader China market dynamics—all contained to less than 10% of revenue. CFO Vitor Roque framed the cadence: “Performance was broad-based, driven by continued double-digit growth in several of our key platforms and strong performance in the U.S., partially offset by a difficult prior year comparison in Alaris and continued pressure in vaccines in China, all consistent with our expectations.” — Vitor Roque, Executive Vice President and Chief Financial Officer · 2026-08-06 Alaris remains the most defined headwind: a 100-basis-point drag this year, stepping to 200 basis points in fiscal 2027, after which the compare rolls off. The company reiterated its confidence in the mid-single-digit algorithm once that window passes.
Management used the momentum to lift guidance, moving revenue growth to the high end of low single digits and raising the adjusted EPS midpoint to $12.62–$12.72. Currency is expected to be a ~100 basis point tailwind. The Alaris flow (as the trajectory tags it) is the key variable in the Q4 step-down, but the underlying portfolio's strength is the headline.
Margin Expansion and the BD Excellence Flywheel
Despite absorbing roughly 110 basis points of tariff impact, adjusted gross margin held at 54.3% and adjusted operating margin at 24.9%, down modestly year-over-year but ahead of internal plans. The driver is the company's operational system. “BD Excellence” is now churning out ~8% gross productivity per quarter, with more than 2,000 Kaizens planned for the year. Management is applying the same lens to commercial and R&D processes, pulling timelines forward by as much as ten months in early projects.
Adjusted EPS was $3.23, up 4.9% and ahead of our expectations, reflecting our strong revenue performance.
Cash flow is also flexing: year-to-date free cash flow rose 45% to $1.7 billion, enabling $3.1 billion in shareholder returns. The company maintains a 2.9x net leverage position and is targeting 2.5x long-term. The quality of the earnings power is visible in the margin trajectory. While GAAP operating margin is temporarily compressed by one-time items, the adjusted path is clear.
The adjusted operating margin of 24.9% is the number management uses to set expectations—GAAP operating margin is heavily distorted by separation and discontinued operations this quarter. The trend over time, however, shows the BD Excellence-driven expansion in the underlying business.The free cash flow swing from a negative Q1 to substantial positive generation reflects improved working capital and lower Alaris remediation outlays.The Road Ahead
The durable story is the reallocation of the whole company around faster-growing, higher-margin categories. The GLP-1 franchise now has ~100 signed agreements across novel and biosimilar molecules, including the first biosimilar launch on the higher-value Vystra pen. New products like the Liverty TIPS stent and the Acumen IQ Plus finger cuff are expanding addressable markets. With Investor Day looming and the Alaris compare rolling off in fiscal 2028, the setup is for an acceleration toward mid-single-digit growth and continued margin expansion.
