BDX Closes the Loop: The Transition Story Becomes an Arithmetic Story
New BD's first clean print pairs a 40% ninety-day re-rating with a deliberately conservative FY27 guide, a U.S. manufacturing deal that is a free tariff option, and an AI platform that finally has a number attached
BDX · Earnings Call · 2026-11-05
The transformation is finally closed — and the language changed with it
For five straight calls, BDX was a company explaining itself: the Waters/life-sciences separation, the Alaris headwind grow-over, a prudent approach after prudent approach to China and vaccines. This quarter the scaffolding comes down. The separation is complete, Alaris remediation is finished, and management has a new noun: "New BD," a focused med-tech company. The tell is in what quietly vanishes from the keyword set — 'Separation' and 'Alaris remediation' were among the biggest movers of prior quarters; now they are gone. “We will be communicating and expect we will be back at mid-single-digit growth, just like we were in FY 2026, ex-Alaris headwind.” — Tom Polen, Chief Executive Officer · 2026-11-05 The transition story has become an arithmetic story. The quarter itself was not a transition quarter: revenue of $5.3 billion, up 5.6% (high single digits ex-Alaris), adjusted EPS of $4.15, up 27.3%. Full-year revenue $19.5 billion and EPS $12.70. The segment reel is unremarkable in the best way — Advanced Patient Monitoring near a two-decade growth high, Rowa pharmacy automation double-digit, PureWick double-digit, and biologic drug delivery up on GLP-1s.Guidance: the same script, a much higher stock
The tension is that none of this is new information to the tape. BDX is up nearly 40% in ninety trading days and sits within roughly 4% of its 2023 high — a name that has re-rated hard into the print. So when Vitor Roque guided fiscal 2027 to low single-digit revenue growth (mid-single ex-Alaris), roughly flat operating margins, and EPS of $13.10–$13.30, the analyst pushback was immediate and familiar: “we want to make sure that we take a prudent approach from our guidance.” — Vitor Roque, Chief Financial Officer · 2026-11-05 That reflex has been present for at least four quarters — Travis Steed asked a near-identical conservatism question back in November 2025, and Larry Biegelsen extracted the exact framework in August: “we have consistently characterized FY '27 as low single-digit revenue growth and that is due to the Alaris remediation coming to an end in FY '26, which creates a 200 basis points headwind next year.” — Vitor Roque, Executive Vice President and Chief Financial Officer · 2026-08-06 So the guide is not a disappointment, it is a promise kept. The new wrinkle is the shape: Q1 EPS of just $2.32–$2.37 against a $13.20 midpoint implies an unusually steep ramp through the year, blamed on Alaris compressing 250bps in Q1 specifically and the APM system cutover from the Edwards platform. “I want to make sure that we have a guidance that is responsible and prudent, and we can deliver during the year with upsides to our shareholders.” — Vitor Roque, Chief Financial Officer · 2026-11-05 It is a reasonable answer — and also exactly what you say when you want the option to beat. The genuine change is cash, not growth
The thing that actually moved this year — and the thing that justifies a higher multiple on a mid-single-digit grower — is free cash flow. Full-year FCF of $2.9 billion, up 42%, with conversion stepping from roughly 57% to 74% and a 90% target out there. “This reflects lower working capital and non-operating cash items, such as Alaris remediation outlays. Including separation costs, free cash flow conversion increased from approximately 57% to 74% in fiscal 2026, and we expect another meaningful step up this year.” — Vitor Roque, Chief Financial Officer · 2026-11-05 $3.6 billion returned to shareholders, a 55th consecutive dividend increase, 2.7x net leverage. This is where the fundamentals and the narrative diverge in an instructive way. On the recast continuing-operations numbers through the June quarter, revenue was $5.0 billion and operating margin sat near 13%, a reminder that the adjusted story is doing a lot of work. More striking, the balance sheet still carries a net debt position near $12.6 billion. And valuation is not cheap on cash: price-to-revenue has recovered to roughly 1.8x from a 4.4x high, but on GAAP free cash flow the multiple stays elevated because FCF is seasonally negative in the June quarter. The bull case is not 'cheap today' — it is 'conversion goes to 90%, and the denominator compounds.'Two new pillars: Washington and AI
The genuinely company-unique development this quarter is the U.S. government agreement, which lands in the keyword set as domestic manufacturing. The payoff is tariff relief: covered products would be exempt from future Section 232 duties, an option management explicitly excludes from guidance. “If and when that is communicated, we'll look to provide an update to our guide at that time.” — Tom Polen, Chief Executive Officer · 2026-11-05 That is a real asymmetry — a free call option sitting outside the numbers, and a rare case where the negotiated outcome (about 80% of Medical Essentials made and sold in the U.S.) doubles as supply-chain insurance. Second, AI has gone from garnish to headline — it is now the company's single highest-momentum keyword. The substance: 100% attachment of the Incada AI platform on new Pyxis Pro placements, the Cognite platform deployed across manufacturing, AI tooling in the sales hiring funnel, and a new CIO imported from Lenovo. “we're seeing 100% attachment rate now on all new Pyxis placements, which is going to be embedding that across healthcare systems quite rapidly.” — Tom Polen, Chief Executive Officer · 2026-11-05 The recurring win rate language — now applied to central-fill (six for six in Q4) and to biologic drug delivery more broadly — suggests this is a selling-machine story, not just a product story.Through this agreement, we committed to invest $19 billion in the U.S. over several years, with $3 billion of the overall investment directed toward manufacturing expansion. Importantly, this does not represent incremental investment for BD.
