Revenue growth of 153% ex-shipping, expanding reimbursement premium, and a new incisionless-surgery society signal a company crossing the chasm.

A Quarter of Timing Headwinds and Underlying Strength

Profound Medical's Q2 2026 headline revenue of $2.5 million looked modest, but the company attributes a significant portion to a $3.1 million shipment that slipped into July. “Excluding the shipment timing, second quarter revenue would have been approximately $5.6 million representing a 153% year over year growth.” — Matthew Sobczyk, Financial/Investor Relations · 2026-08-06 This timing issue is a recurring theme—management flagged similar logistics challenges in prior quarters and is adding operations staff to smooth the runway. Beyond the timing, the quarter highlighted real commercial momentum. The Index 20 measure of same-store utilization grew 39% in the first half of 2026 versus 2025, and the company announced a record month for new orders in July. The qualified sales pipeline now stands at $70 million, split roughly 90/10 between TULSA-PRO and Sonalleve. In addition, the install base grew to 84 systems, up from 80 at the end of Q1, even as a few sites paused during the transition from placement to capital ownership.

Reimbursement Premium Widens — A Key Tailwind

Tom Tamberrino, Chief Commercial Officer, detailed the CMS proposed rule for 2027. TULSA remains at APC Level 7, with OPPS payment increasing 14.9% to $15,500, versus $10,800 for HIFU/Aquablation and $12,300 for robotic RP. He summarized:

To summarize those proposed changes, Talsa remains at urology APC Level 7, with OPPS payment increasing 14.9% to $15.5 thousand per procedure. That compares to an 11.6% increase for HIFU and Aquablation to $10.8 thousand and $12.3 thousand for Robotic RP.

Thomas Tamberrino, Sales/Business Development Executive · 2026-08-06
The widening reimbursement gap, combined with expanding covered lives—another 18.3 million added in Q2, bringing the total to roughly 30 million—strengthens the economic case for hospitals to adopt TULSA. The addition of employer-sponsored health plans like Johns Hopkins and Prime Healthcare underscores broader payer acceptance. On the physician side, TULSA is also more competitive on an apples-to-apples basis, with $880 per procedure versus $865 for HIFU and $1,060 for robotic RP.

Commercial Inflection and the New Society

The company is also leaning into a broader industry shift. At SRS-26, ProFound was a founding member of the new Society of Incisionless Surgery (FIS), positioning TULSA as the only MRI-guided, autonomous robotic platform. As Arun Menawat put it, “The time for incisionless surgery has come.” — Arun Swarup Menawat, CEO · 2026-08-06 This narrative is supported by record order activity and the 70%+ gross margin, which the company expects to hold for the year. TULSA is the only modality that offers both whole-gland and focal ablation, and with the new Focal therapy options, the platform is versatile across the prostate disease spectrum. Prior quarters set the stage for this acceleration. In Q1 2026, management noted commercial payers were paying 1.5x to 2.5x Medicare rates, reflecting early traction with private insurers. “The typical payments are between -- I would say, most of them are between 1.5x to 2.5x of Medicare.” — Arun Menawat, Chief Executive Officer · 2026-03-06 And in late 2025, Tom highlighted that Medicare reimbursement was "coming through exactly as we thought it would," fueling utilization. “The answer is yes. And what I can tell you is that the tailwinds that are coming online are the fact that the CPT1 coding that we've referred to and the reimbursement associated with that through Medicare is coming through exactly as we thought it would.” — Thomas Tamberrino, Chief Revenue Officer · 2025-11-13

What to Watch

The key catalysts remain the 5-year CAPTAIN data readout expected in Q4, the Siemens Free.Max integration (FDA clearance anticipated early next year), and continued expansion of payer coverage. With cash of $38.3 million and a path to profitability at the $80-85 million revenue range (current run-rate is ~$25 million), Profound is positioning itself at the forefront of incisionless, MRI-guided prostate therapy. The quarter was a reminder that revenue recognition timing can obscure strong underlying demand. But the record pipeline, expanding reimbursement, and the creation of a dedicated society for incisionless surgery all point to a company crossing the chasm.
