The engineered valley

Around the midpoint of a deliberately buried year, Treace Medical's story has turned from defense to scheduling. Second-quarter revenue fell 4% to $45.4M, a decline management attributed to "lower procedure kit sales to stocking distributors and a continued mix shift towards lower-priced minimally invasive products." But the framing was unmistakably offensive: “we accelerated our case volume growth. We talked about mid-single digits over the last couple of quarters. The market has already priced that calendar: TMCI has rallied roughly 140% over the last 90 trading days, a +133% move across ten weeks, even as the quarterly income statement still shows a $15.9M net loss. This is not a company reporting a turnaround; it is a company reporting the schedule of one.

Adoption is now measurable

The heart of the bull narrative is numeric progress on surgeon adoption. Roughly 40% of the Lapiplasty surgeon base has now used at least one of the three bunion systems launched in Q3 2025, up from 35% in the first quarter — and, critically, the new-cohort cascade is reaching the legacy franchise. The mix hand-off, on schedule

The persistent drag is the ASP mix: new MIS osteotomy systems carry lower price points than Lapiplasty, so higher case counts do not immediately lift the top line. But the headwind has a fixed expiry. The midfoot platform changes the basket

The most distinctive development was forced quantification of the adjacency story. SuperBite compression screw entered limited release in Q2 and fully commercializes this quarter; HyperPlate XM and GreatReleaseXM round out the midfoot/hindfoot fusion toolkit, with CartiBlaster arriving in Q4. On the quarter, John put a fresh number on the expansion:

That is the first time the company has sized the foot-and-ankle expansion beyond bunions, and it recasts the conversation from addressable market saturation to wallet share in adjacent procedures — each incremental SuperBite case often pulls additional Treace products with it.

Cash bends with the trough

The model only works if the cash burn lands inside the guide, and the quarter supported it: cash usage fell 57% year-to-date and the ~50% burn reduction for the year was reaffirmed. Free cash flow (ex-SBC) tells the same story, turning from a deep structural drain toward the break-even management has telegraphed for 2027. On the equity side, Price to Revenue had already compressed to 0.4x, so the ten-week rally is fundamentally a re-rating on the scheduled inflection rather than a recovery off distressed levels. The full plan now hinges on a sequence: lap the mix, ride Lapiplasty Lightning's Q4 limited launch, and convert a 40%-and-climbing adoption base plus a $300M adjacency into the strongest quarter of the year. The company has given investors a date, and the tape has given it a standing ovation in advance.
