UroGen's Zosturi Beat Meets an Inflection That Isn't
The launch is real—$68 million in quarterly Zosturi revenue—but management just told investors the next leg is a 2027 story, and JELMYTO is moving the other way.
URGN · Earnings Call · 2026-11-04
A beat the tape already discounted
UroGen's third quarter was, on the surface, exactly what the bull case needed. Total revenue reached $89 million versus $27.5 million a year earlier, and Zosturi produced $68 million of net product revenue, up 35% sequentially. Chris Degnan told investors “we anticipate achieving profitability and generating positive cash flow in the fourth quarter of 26.” — Christopher Degnan, Chief Financial Officer · 2026-11-04 That is a remarkable turn for a company that was burning cash at a double-digit quarterly rate not long ago. Yet the stock is down 21.9% over the last 90 trading days and 42.9% from its August peak. The market is not arguing with the quarter; it is arguing with the slope after it.
The reason is simple: management used the call to lower the ceiling on near-term expectations. Liz Barrett was unusually direct when asked about an inflection:
I do not think you will see an inflection point in the next few quarters. ... We are going all in on DTC. In 27. That takes time, so it is not likely to be, you know, before kinda mid-year 2027.
That is not a new caution—on the August call she said “I would not say acceleration, at least not in Q3” — Elizabeth Barrett, Chief Executive Officer · 2026-08-05—but the stock had already rallied hard into the summer, and the tape is now repricing the wait. The inflection point has been pushed from a hope to a 2027 event.
Zosturi: depth before breadth
The operating metrics show why the next leg is not automatic. Zosturi's growth is coming from depth within existing accounts, not yet from broad adoption. Unique prescribers rose to 662 from 452, and repeat writers now represent 50% of total writers, up from 45% in Q2. More than 10% of the roughly 6,500 core-target urologists have prescribed, against a total universe of over 8,000. That is real progress, but it also means the unique prescribers base is still small relative to the opportunity. Management is explicit that the revenue is being driven by the depth of existing users, not the breadth of new ones.
That is why the company keeps emphasizing new prescribers and repeat utilization. The commercial message that lands is not a clever marketing line; it is the clinical result plus reimbursement confidence. Barrett described the barriers plainly: “the operational lift and the reimbursement lift are the biggest barriers” — Elizabeth A. Barrett, Chief Executive Officer · 2026-11-04. Physicians try one or two patients, get paid, then expand. That is a slow conversion funnel, and it is why management still will not issue formal Zosturi guidance. The prior call said it would wait: “we think it's prudent to allow the demand trends to play out through the rest of this year before we consider introducing formal guidance for ZUSDURI.” — Christopher Degnan, Chief Financial Officer · 2026-08-05 Nothing in this quarter changes that posture. Degnan would only say “we do expect continued growth, and we are comfortable with where consensus sits for the full year for Zosturi.” — Christopher Degnan, Chief Financial Officer · 2026-11-04
JELMYTO is the crack in the story
The real negative was JELMYTO. Net product revenue fell to $21 million from $22 million, including a $1.5 million unfavorable gross-to-net adjustment from the CMS discarded-drug provision. More important, UroGen cut full-year JELMYTO guidance to approximately $90 million, citing competition for patients from clinical trials in low grade UTUC. Barrett did not sugarcoat it: “we really did get hit with the clinical trials” — Elizabeth A. Barrett, Chief Executive Officer · 2026-11-04. That is a visible reversal from the prior call, when she described JELMYTO as a predictable low-single-digit grower: “We will continue to see low single-digit growth. That's what we've been talking about.” — Elizabeth Barrett, Chief Executive Officer · 2026-05-06
The JELMYTO cut matters because it removes the cushion. Zosturi is the growth engine, but JELMYTO was supposed to provide a stable base while the launch scaled. Instead, it is shrinking, and the company is forming a strike team to defend its heavy users. The balance sheet makes the next few quarters less forgiving: Effective net cash has fallen to -$62 million, even as management points to $90.5 million of cash and $117 million of accounts receivable. The profitability promise is credible only if Zosturi keeps compounding and the receivables convert.
The pipeline is 2027-2028 optionality
The call also spent real time on the future. UGN-103's NDA was accepted with a PDUFA date of June 14, 2027, and the newly listed patent is expected to cover Zosturi and UGN-103 into July 2044. Mark Schoenberg said “the 12-month durability data will be will be in incredibly important to the label” — Mark Schoenberg, Chief Medical Officer · 2026-11-04, and management plans to file for a permanent J-code by July 2027 for a January 2028 effective date. That is a known catalyst type—the market has been rewarding PDUFA target action date stories—but it is still seven months away and the commercial handoff is a 2028 event. Liz Barrett made the sequencing clear on the March call: “the plan would be to not introduce it into the market until after we get a permanent J code.” — Elizabeth A. Barrett, President and Chief Operating Officer · 2026-03-02
The newer piece is the strategic collaboration with IntraGel, which brings a biodegradable sustained-release platform and an option on a cisplatin gel therapy for advanced head and neck cancer. It broadens the story beyond uro-oncology and adds a High grade NMIBC trial starting in Q4 2026, plus UGN-501 data expected in 2027. Strategically sensible; commercially distant. None of it resolves the current debate about whether Zosturi can accelerate before the DTC campaign arrives in mid-2027.
What the tape is saying
At 9.5x revenue after the drawdown, UroGen is not priced as a broken launch. Price-to-revenue is 9.5x; that is a show-me multiple for a company with a genuine new product and a 2028 pipeline. The quarter proved Zosturi is real. The stock is telling management that the next leg has to be visible in the numbers, not in a 2027 DTC plan. Until repeat prescribers convert into broader prescribing and JELMYTO stops leaking, the market will treat every beat as a quarter, not an inflection.
