Zimmer Biomet's Beat-and-Raise: Sales-Force Transformation and Innovation Cycle Bite

The Quarter: A Beat-and-Raise Built on Execution
Zimmer Biomet reported second-quarter 2026 net sales of $2.177 billion, up 4.8% reported and 4.0% organic constant currency, beating expectations and prompting a full-year organic revenue guidance raise to 2.25%–3.25% from 1%–3%. Adjusted EPS of $2.07 held steady year-over-year despite Paragon 28 dilution and heavy investment in the U.S. commercial channel, and management increased the bottom-line outlook as well. The strength was broad-based: U.S. organic growth hit 4.6%, with Hips delivering 5.9% growth. Ivan Tornos credited the company's comprehensive suite of solutions and the ongoing go-to-market overhaul.
“In the U.S., our hip triple play continues to gain momentum, driven by the continued penetration of Z1, our triple-taper hip stem, which now represents over 40% of our U.S. hip stems and will soon surpass 100,000 implants worldwide.” — Ivan Tornos, Chairman, President and CEO · 2026-08-05
The raise is no accident — it reflects a deliberate philosophy shift. After last year's public misstep on guidance, Ivan emphasized a "say less, do more" discipline, but the evidence now supports confidence. As he noted in the closing remarks:
The transformation of the company is going as expected, if not better than expected. We are extremely confident on the guidance raise that we provided this morning.
Under the Hood: S.E.T., Hips, and Technology
The out-performance is not just the core Recon market; it's the higher-growth segments that are now pulling their weight. S.E.T. accelerated 180 basis points sequentially to 3.4% organic constant currency, with Paragon 28 growing mid-teens, upper extremity up upper-single-digit, and CMFT double digits. The real star, however, is Technology. CT scan and non-CT imaging are both part of a full robotics and navigation portfolio that drove record capital sales. Ivan highlighted:
“The lion's share of the growth is Technology. Technology in the U.S. grew 30% in Q1. In the second quarter grew 53%.” — Ivan Tornos, Chairman, President and CEO · 2026-08-05
ROSA Shoulder, the only robotic system capable of both anatomic and reverse procedures and both humeral and glenoid resection, is entering full market release after a successful limited release. The company also sees a robust pipeline for its next-gen high growth categories. Outside the U.S., the iodine-coated hip launch in Japan is exceeding expectations — supply is struggling to match demand, and the company is already talking to the FDA about a U.S. pathway. This is the kind of disruptive innovation that shifts the WAMGR trajectory.
Financials Reconfirm the Pivot
The fundamentals support the narrative, even though the latest filed 10-Q (Q1 2026) lags the call. Revenue grew ~9% yoy in Q1, and operating margin expanded to 17.9% from 15.3% a year ago, reflecting operational leverage despite the commercial investment. The company's free-cash-flow engine remains intact, with Q2 operating cash flow of $448M and FCF of $308M, up 18% and 24% yoy respectively — even as management expects to invest behind the U.S. sales transformation and new product launches.
Operating margin expanded to 17.9% in Q1 2026 (from 15.3% yoy), though management guided FY26 operating margins down a little more than 50bp as they continue to reinvest in the U.S. commercial organization.
We also see deliberate capital allocation: $500M of buybacks in H1 2026, with plans to repurchase up to $1B this year. The balance sheet remains manageable; net debt is ~$7.0B, but interest coverage is improving and cash flow supports the plan.
The Path to 2027: Monogram, CFO, and the End of Transition
The bigger story is what's next. The Interim CFO search is ongoing, but that's secondary to the product pipeline. The Monogram 510(k) submission is imminent, and a launch is targeted for early 2027. Management is hiring over 200 reps behind it and positioning it as a step change in robotics — autonomous or semi-autonomous, with cases under four minutes. Meanwhile, the U.S. sales-force transition is "going better, if not much better than expected," with attrition lowest in years and productivity improving in converted territories. Ivan reiterated:
“The work that we are doing to transform our company, starting with our critical commercial channel is well underway.” — Vijay Kumar, Analyst · 2026-08-05
This is a company that has been in a -42% drawdown from its 2021 peak but has now posted a 90-day +8.4% rally. The beat-and-raise, combined with a visible 2027 catalyst stack, makes this a genuine inflection story. The key risk remains execution of the sales-force changes and the China/emerging-market drags, but the guidance raise is a strong signal that management sees the second half as accelerating.
“We expect bigger growth in the second half. So we like where we are with S.E.T. We do have 2 headwinds... If you take those out, the U.S. growth will be solidly in the upper single digit year-to-date.” — Patrick Wood, Analyst · 2026-08-05
The contrast between this quarter and the prior-year guidance wobbles is stark — and it suggests the strategic pivot is starting to pay off. Investors will be watching whether the innovation platform and the rebuilt sales force can turn 4% growth into 5%-plus sustainably. Today's print argues yes.
